Ergonomic evaluations are completed for on-campus employees that require specific modifications to their work environments. Please use the information below to submit an ergonomic evaluation request. Once the form is received by Operations it is then rerouted to Risk Management and an Ergo Evaluator is contacted. The Evaluator will then contact the Employee and conduct the evaluation. Operations will receive the evaluation, order the recommended items and/or make any recommended changes to the office space. With the exception of ergonomic chairs, items are typically delivered within two weeks. The process may be prolonged if IT needs to be involved in the installation of an ergo item.
